Defect Lifecycle

States chuẩn

New → Assigned → In Progress → Fixed → Verified → Closed

Roles per state

  • New — QA report.
  • Assigned — Lead triage.
  • In Progress — Dev fix.
  • Fixed — Dev mark, push to QA env.
  • Verified — QA confirm fix.
  • Closed — Lead close.
  • Reopened — fix fail verify → back to In Progress.

Triage process

  • Daily / 2x weekly bug triage meeting.
  • 15-30 phút.
  • QA Lead + PM + Dev Lead.
  • Decide: severity, priority, owner, target release.

Severity vs Priority

  • Severity — technical impact (crash > broken feature > UI glitch).
  • Priority — business urgency (revenue blocker > customer ask > nice-to-have).
  • Có thể: Critical severity + Low priority (crash khi 1 user dùng dark mode + small screen + offline + edge case → fix sau).

Aging report

  • Bugs open > 30 days: review weekly.
  • Bugs open > 90 days: escalate.
  • Bugs > 180 days: closed-deferred or back to backlog.

SLA per severity

  • Critical: 4h response, 24h fix.
  • High: 24h response, 1 week fix.
  • Medium: 3 days response, 1 month.
  • Low: backlog.

JIRA + Xray. Linear cho startup nhanh. GitHub Issues cho OSS.

Metric track

  • Open bug count by severity (trend).
  • Time to fix (median, p90).
  • Reopened rate (target < 10%).
